1. Find the Lions Club for the area of your event. Please visit the District 33K Club Information page for more information on the geographic coverage, leadership, and meetings of each Lions Club.

  2. Ask the Lions Club to officially host/sponsor your event.

  3. After receiving approval from the Host Lions Club, please ask a Lion to complete the Booking Request form.

  4. Please be patient! We will try our best to get back to you within ~2 business days. If you haven’t heard from us beyond that time, then please feel free to submit a query using our contact form.

The following Booking Request form is for Lions members only. If you would like to partner with the Eyemobile but are not a Lion yourself, then please see above.

  • Unfortunately, the Eyemobile is not currently handicap accessible. Please make suitable alternate arrangements if handicapped clients are expected.

  • Contact person will be notified of driver’s name and phone number ~2-3 weeks prior to the date of the event
