Please fill out this form to reserve the Eyemobile

Please note: The Eyemobile is available for use outside of District 33K provided no District 33K Club or organization wishes to reserve it on the same date.

All Eyemobile Screenings must be sponsored by a Lions Club.  If you are not from a Lions Club, please visit our District website at http://www.lions-33k.org for your local Lions Club contact information.   Please fill out reservation form prior to contacting Club, or if there is no Lions Club in your area.

Clubs:  Please review document (click here) for time frames to verify all Eyemobile Screening information.  Please use timetable to verify drivers, staffing, etc.
